About the Role:
North Point Senior Homecare in Virginia Beach, VA is looking for a compassionate and skilled PRN Licensed Practical Nurse to join our dedicated team. Responsibilities:
Provide direct patient care to senior clients in a home health setting on a PRN (as-needed) basis
Administer medications, injections, and treatments as prescribed by the physician
Monitor and document patient vital signs, health status, and progress
Assist with wound care, catheter care, and other skilled nursing procedures
Communicate effectively with patients, families, and the care team regarding patient needs and changes in condition
Develop and follow individualized patient care plans in coordination with RNs and physicians
Ensure accurate and timely clinical documentation in compliance with state and agency regulations
Requirements:
Active Licensed Practical Nurse (LPN) license in the state of Virginia
Prior home health, geriatric, or senior care experience preferred
Strong clinical assessment and critical thinking skills
Ability to work independently with minimal supervision in a home care environment
Excellent communication and interpersonal skills with patients and families
Valid driver's license and reliable transportation required
CPR/BLS certification required or ability to obtain upon hire
