Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Observe and document patient conditions, progress, and any changes in health status
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed
• Communicate effectively with patients, families, and the broader care team regarding treatment plans
• Ensure compliance with all state, federal, and agency regulations and standards of care
• Provide patient and family education on medications, treatments, and home health management

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of Georgia
• Current CPR/BLS certification required
• Prior home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of medication administration, wound care, and patient assessment
• Excellent communication, organizational, and time-management skills
• Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
• Compassionate, patient-focused demeanor with a commitment to quality care
• Pediatric Experience is a plus
