Pay Rates – Per Completed Visit
nPer Visit - $42
nDowntime (pre-approved wait time) - $32/hour
n"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do
n- n
- Provide skilled occupational therapy treatments to home health patients under the supervision of an Occupational Therapist in accordance with the established plan of care n
- Assist patients in improving their ability to perform activities of daily living including dressing, bathing, grooming, meal preparation, and home management n
- Implement therapeutic interventions focused on fine motor skills, coordination, cognitive function, and adaptive techniques n
- Monitor and document patient progress toward established therapy goals n
- Educate patients and families on home exercise programs, adaptive equipment, and compensatory strategies n
- Maintain accurate and timely documentation in the EMR within required timeframes n
- Communicate regularly with the supervising OT and interdisciplinary care team regarding patient status and progress n

Requirements
nMust-Have:
n- n
- Active COTA license (Ohio) + current BLS certification n
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area n
- Minimum 1 year of clinical occupational therapy assistant experience n
Preferred / Nice-to-Have:
n- n
- Prior home health experience n
- Experience working under OT supervision in a community or home-based setting n
- Familiarity with HCHB or similar home health EMR n
- Experience with adaptive equipment recommendations and home modification assessments n
