PRN Case Manager – Inpatient Discharge & Care Coordination

Select Specialty Hospital, part of Select Medical, seeks a Part-Time/PRN Case Manager to support discharge planning, utilization reviews and care coordination for chronically critically ill patients. Responsibilities include coordinating with payors to maximize coverage and leading multidisciplinary teams.

Qualified candidates have current licensure (RN/LPN/LVN, RT or SW) and prior inpatient case management experience in acute settings. Shifts are weekday, 8a‑5p, as needed.
