ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on delivering affordable, VIP care to seniors, aiming to transform healthcare for this population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, working to build trusting relationships and influence patient behaviors.

Responsibilities
- Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
- Engage with the hospitalist when a patient is hospitalized
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Lead the care team for patients able to come to the office
- Have an active role in the management of their center and help cover for other providers
- Take an active role in recruiting patients for the center and additional providers for the company
- Adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures
Compensation
- PAY RANGE: $221,141 - $315,915 Salary
- This position may also be eligible for bonuses or commissions

Requirements
- Ability to function both independently and in collaboration with other health care professionals
- Willingness to continuously work to develop and improve leadership skills
- Willingness to work with care teams towards achieving high quality outcomes and quality measures
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes
- Fluency in English, verbal and written
