$232,506 - $348,758 base + bonus opportunity Monday-Friday schedule | 8:00 AM - 5:00 PM Outpatient-only primary care Quarterly Incentive Plan (15% of quarterly earnings) Long-Term Incentive Eligible — physicians may participate in the Mosaic Health Profit Shares program, designed to reward physicians for contributing to the organization Day-one PTO (160 hrs), full benefits, 401(k) match, CME (5 days + 2K reimbursement)
