Primary Care Physician (PCP)

ChenMed is a physician‑led, mission‑driven primary care organization that honors seniors with affordable VIP care. We are expanding rapidly and need dedicated physicians to join our team.

Responsibilities

• Provide primary care for up to 450 patients per panel, ensuring high quality, cost‑effective, evidence‑based care.
• Act as a health coach, setting short‑ and long‑term goals, partnering with patients, and following up to improve health outcomes.
• Simplify and prioritize care for actionable behavior change and lead the care team (medical assistants, coordinators, facilitators).
• Manage acute and chronic illnesses, coordinate care with hospitalists, transitional‑care teams, specialists, and community resources.
• Maintain accurate documentation, participate in quality improvement, risk‑management, and clinical meetings.
• Support center management, cover for other providers, recruit new patients and physicians as needed.

• Up‑to‑date with evidence‑based medicine, nutrition, and lifestyle interventions.

• Self‑care for physical, mental, social, and spiritual wellness.
• Proficiency in Microsoft Office Suite.
• Willingness to travel locally and regionally for training and recruiting.
• Fluent in English (other languages may be required depending on location).
• Use independent judgment in patient care decisions.

Pay Range

$221,141 - $315,915 Salary
