Responsibilities
- Provide comprehensive outpatient primary care services to adult and geriatric patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Develop individualized care plans that promote preventive care, wellness, and disease management
- Coordinate care with specialists, hospitals, and interdisciplinary care teams
- Educate patients and caregivers on treatment plans, preventive health, and healthy lifestyle choices
- Maintain accurate and timely medical documentation within the electronic medical record (EMR)
- Participate in quality improvement initiatives focused on value-based care and positive patient outcomes
- Build strong, long-term relationships with patients and their families through compassionate, patient-centered care

Shift & Schedule
- Full-time
- Monday through Friday
- Daytime outpatient hours
- Minimal call requirements, depending on practice location
Requirements
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Active Florida medical license (or eligible for licensure)
- DEA license (or eligible)
- Current BLS certification
- Experience with Medicare Advantage, managed care, value-based care, or geriatric populations is preferred but not required
- Bilingual (Spanish) skills are preferred for select locations
