ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, acting as a health coach and leading a dedicated care team.

Responsibilities
- Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
- Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
- Engages with the hospitalist when a patient is hospitalized
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Leads a care team for patients able to come to the office
- Engages with the transitional care team and other resources for patients unable to come to the office
- Actively manages their center and helps cover for other providers
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP
Compensation
- $214,700 - $306,714 Salary
- Metrics regularly measured to help PCP’s become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures

Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility is required
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
