Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.
Responsibilities Include
- Assessing, diagnosing, and managing patient care
- Prescribing medications and treatments
- Maintaining electronic medical records
- Collaborating with leadership to meet quality goals
- Leading and managing care teams in coordination with Practice Managers
- Performing other assigned duties
Qualifications
- MD or DO graduate with Board Certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted medical license and DEA registration
- US work authorization
Preferred Skills & Experience
- Geriatrics fellowship or experience in geriatric care
- Experience in value-based healthcare and outpatient primary care
- Collaborative practice experience and bilingual skills, where applicable
