We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director.The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records.
Refers the patient to specialists as needed.
