- Experienced integrated behavioral health professional embedded within the practices
Responsibilities
- General outpatient family/internal medicine primary care
- Work collaboratively with a multi-disciplinary team to create individualized treatment plans for patients
- Caring for a wide range of patients
- Minimal call - telephone only
Compensation
- Sign On Bonus Available to External Candidates - Up to $100,000
- 24-month salary guarantee

- $100,000 sign-on incentive with no repayment clauses for eligible external candidates
- The salary range for this role is $226,000 to $366,000 per year
- Salary Range is defined as total cash compensation at target
- The actual range and pay mix of base and bonus is variable based upon experience and metric achievement

Requirements
- M.D. or D.O.
- Must be board certified or Board Eligible in Family Medicine, Internal Medicine, or Med-Peds
- Active Oregon Medical License or ability to obtain prior to employment
- Active DEA or ability to obtain prior to employment
