ChenMed is seeking a dedicated Primary Care Physician, Partner (PCPP) to join their rapidly expanding, physician-led organization. This role involves providing VIP care to seniors, focusing on outcomes, and acting as a health coach to improve patient well-being. The PCPP will be instrumental in transforming healthcare for the neediest population while contributing to a positive center culture.
Practice Info
- Physician-led and mission-driven primary care organization
- Focus on full-risk Medicare Advantage providers
- Accountability for outcomes, strong clinical care, and cost-effectiveness for a risk-adjusted panel of at least 400 patients
- Utilizes between 18-21 appointment slots per day (new patients count for 2 slots, follow-up patients 1 slot)
- PCPPs are leaders in the organization and centers, expected to champion a positive culture of love, accountability, and passion
- Works closely with managers and medical directors to ensure compliance with guidelines
Responsibilities
- Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
- Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed care
- Engages with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office
- Engages with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)
- Has an active role in the management of their center and helps cover for other providers
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with, an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP

Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Current, active MD licensure in State of employment is required
- A minimum of 6 years clinical experience in geriatric, adult or family practice setting, including 2 years of ChenMed PCP, Senior Partner experience required
- Board Eligibility is required
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Once Board certified will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
