Primary Care Physician

ChenMed is seeking a Primary Care Physician (PCP) to join their rapidly expanding team, dedicated to transforming healthcare for seniors. This role involves providing direct patient care, conducting assessments, and developing comprehensive care plans primarily within an ambulatory health care center setting. The PCP will play a key role in a clinical operations team, focusing on quality, service, and teamwork to improve the health and well-being of the patient population.

Practice Info

• Functions independently as a primary care practitioner as part of a patient care team
• Provides direct patient care and assessments primarily in the ambulatory health care center setting, or occasionally in acute care, nursing homes, skilled nursing facilities (SNF) and home settings
• Collaborates with members of the multidisciplinary team to ensure patient management strategies are successful

Responsibilities

• Perform geriatric assessment, medical history, physical exam, diagnosis and treatment
• Develop the plan of care, provide health education, and make specialty and case management referrals
• Document clearly according to ChenMed standards for quality, service, productivity and teamwork
• Participate in clinical rounds and conferences, including in-depth documentation through written progress notes and summaries
• Assess acute and non-acute clinical problems independently
• Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, considering the patient’s cultural background, level of understanding, personality and support systems
• Manage patients by writing admission, transfer and discharge orders; ordering and interpreting appropriate laboratory and diagnostic studies; ordering appropriate medication and treatments; and referring patients for consultation when indicated
• Facilitate patient/family conferences to review treatment goals, optimize resource utilization, provide family education and identify needs

Requirements

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
• Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
• Board Eligibility is required
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
• Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
• Must have a current DEA number for schedule II-V controlled substances
• Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
