ChenMed is seeking a Primary Care Physician (PCP) to join their rapidly expanding team, focusing on transforming healthcare for seniors. This role involves providing direct patient care and assessments primarily in an ambulatory health care center, with occasional assignments in acute care, nursing homes, skilled nursing facilities, and home settings. The PCP will be a key part of the clinical operations team, responsible for comprehensive geriatric assessments, diagnosis, treatment, and developing patient care plans.

Responsibilities
- Provide direct patient care and assessments, including geriatric assessment, medical history, physical exam, diagnosis, and treatment
- Develop the plan of care, provide health education, and make specialty and case management referrals
- Ensure clear documentation according to ChenMed standards for quality, service, productivity, and teamwork
- Participate in clinical rounds and conferences, including in-depth documentation through written progress notes and summaries
- Function independently as a primary care practitioner as part of a patient care team
- Independently assess acute and non-acute clinical problems
- Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, considering cultural background, understanding level, personality, and support systems
- Manage patients by writing admission, transfer, and discharge orders; ordering and interpreting appropriate laboratory and diagnostic studies; ordering appropriate medication and treatments; and referring patients for consultation when indicated

Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility is required
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
