Practice Info
- 100%, office-based, outpatient Primary Care setting
- Primary practice location based in Newington with supplemental clinical time in West Hartford, Connecticut
- Coordinated, team-based care model supported by nursing and dedicated Medical Assistants
- Streamlined access to Behavioral Health, High Risk Nurse Care Managers, and Social Workers
- Equipped with EPIC, Dragon, Ambient AI integration, HIPAA-compliant texting, patient self-scheduling, and patient portal
