Position Overview

The Primary Care Physician (PCP) is responsible for delivering high‑quality, cost‑effective care to a panel of up to 450 patients in an outcomes‑focused, value‑based setting. The role emphasizes patient accountability, health coaching, and simplifying action steps for behavior change.

Core Competencies

  • Accountability for Outcomes – Demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in the panel.
  • Coaching for Health – Acts as a health coach, helping patients set short and long‑term health goals and following up regularly.
  • Simplifying for Action – Simplifies and prioritizes appropriately so that behavior change becomes actionable for both patients and care teams.
  • Performance Metrics – Delivers on metrics such as patient admissions per thousand, appointment slot usage, CGCAHPS, clinical gap closures, and medical cost measures.
  • Team Leadership – Leads a care team consisting of a medical assistant, care facilitator, and care coordinator; collaborates with hospitalists, case managers, and transitional care teams as needed.
  • Center Management & Recruitment – Plays an active role in center management, covering for other providers, and recruiting patients and additional providers.
  • Advanced Practice Practitioner Oversight – Supervises and collaborates with APPs, remaining accountable for their actions.

  • Availability and accessibility to patients, including walk‑in visits and timely communication.
  • Strong service orientation and evidence‑based medicine, staying updated on clinical, nutritional, and lifestyle interventions.
  • Leadership development and continuous improvement of practice and center operations.
  • Quality focus and systematic improvement of care outcomes.
  • Influence and collaboration with patients, teams, and organizational stakeholders.
  • Self‑care and physician wellness for sustainable practice.
  • Proficiency in Microsoft Office Suite and related software.
  • Travel willingness (local, regional, national as required).
  • Fluency in English (verbal and written); additional language skills may be required for some centers.
  • Independent judgment with minimal supervision, supporting decision‑making and project leadership.

Education & Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Current State Medical License for the state(s) of practice.
  • Board certification preferred; board eligibility required; ongoing maintenance of certification.
  • Current DEA number for schedule II–V controlled substances.
  • Basic Life Support (BLS) certification within 90 days of employment.

Compensation & Benefits
