Responsibilities
- Provide comprehensive primary care services to Medicare Advantage and senior patients age 65+
- Perform thorough assessments and develop individualized care plans
- Manage chronic conditions, preventive care, and acute medical concerns
- Deliver patient-centered care within a managed care/value-based care model
- Collaborate with physicians, specialists, and interdisciplinary care teams to coordinate treatment plans
- Identify and close care gaps while supporting quality and outcome initiatives
- Educate patients and caregivers regarding disease prevention, healthy lifestyle choices, and long-term wellness
- Build strong relationships with patients and families through compassionate, personalized care
- Maintain accurate and timely documentation within the EMR
Compensation

- Salary: $260K ++

Requirements
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred
- New Graduates Welcome
- Florida Medical License preferred or eligible
- ACN licensed accepted
- Geriatric care experience preferred
- Strong communication and patient engagement skills
- Spanish-speaking ability preferred for one opening
