ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on delivering affordable, VIP care to seniors, aiming to transform healthcare for the neediest population. - Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
- Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals.
- Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable.
- Outcomes-focused, value-based organization with regularly measured metrics including patient admissions/thousand, appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures.
- PCPs are leaders in the organization and centers, expected to help champion a positive culture of love, accountability, and passion along with center leadership.
- The PCP will work closely with applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
Responsibilities
- Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- Takes full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
- Engages with the hospitalist whenever one of their patients is in the hospital.
- Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
- Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
- Engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources for patients that are unable to come to the office.
- Has an active role in the management of their center and will help cover for other providers who may be out for various reasons.
- Takes an active role as needed in recruiting patients for the center and additional providers for the company.
Compensation
- PAY RANGE: $221,141 - $315,915 Salary
- This position may also be eligible for a bonuses or commissions.

- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company.
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes.
