We are looking for a Primary Care Physician to join our team full-time at CenterWell Primary Care . This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.
You will work on-site at our Parkview clinic in Surprise, Arizona. You will report to our Associate Medical Director.

Use Your Skills To Make An Impact
- You will deliver comprehensive outpatient care to adult and senior patients.
- You will maintain accurate medical records and documentation.
- You will diagnose and manage moderately to complex medical conditions.

- You will participate in clinical quality improvement projects.
- You will work with interdisciplinary teams to ensure holistic care.
- You will contribute to projects and innovations in care delivery.
Required Qualifications
- MD or DO from an accredited medical school.
- Active, unrestricted medical license in the state of practice.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- If selected for this role, you will be required to be screened for TB
Preferred Qualifications
- 2 years' experience in value-based care or managing high-acuity geriatric patients.
