Primary Care Physician (PCP) Responsibilities

  • The PCP independently provides primary care for seniors with acute and chronic illnesses.
  • Fully accountable for patient care, outcomes, and care coordination, while seeking specialist consultation when necessary.
  • Engages with hospitalists for patients in hospital settings, regardless of affiliation.
  • Manages assessment, diagnosis, treatment, education, health promotion, and documentation for patients with complex chronic needs.
  • Leads a care team consisting of a medical assistant, care facilitator, and care coordinator for office‑bound patients.
  • For patients unable to visit the office—such as those in hospitals, SNFs, LTC facilities, or homebound—the PCP collaborates with transitional care teams, case managers, and other resources available in the market.
  • Participates in center management, covers for other providers when needed, and assists with recruiting patients and additional providers.
  • Supervises, collaborates with, or functions within a collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions while employed by ChenMed.
  • Performs other duties as assigned by management.

  • Fluency in English, verbal and written; additional language requirements, when applicable, will be specified during application.
  • Demonstrated independent judgment in clinical decision‑making.

Education and Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Current, active state medical license in the state(s) where the candidate will practice.
  • Board certification (or board eligibility) in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred.
  • After board certification, maintenance of certification through MOC, CME, or board examinations as required.
  • Current DEA number for schedule II‑V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.

Pay Range

Salary: $214,700 – $306,714.
