Primary Care Physician, Leadership Fellow

The Primary Care Physician, Leadership Fellow will be part of an accelerated development program for new Physician & Physician Leaders dedicated to patient-centered, value-based healthcare.

Relocation and commencement incentive packages up to $100,000 are available.

Essential Job Duties / Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Takes full accountability for patient care and outcomes and appropriately seeks consultation from internal or external specialists when needed.
  • Once mastering the Core Model, plays an active role in managing their center, the center’s physicians, and covering for other providers as requested.
  • Engages in facilitated practice and leadership rounding both as a recipient and to support growth of other providers in the center.
  • Engages with hospitalists when one of their patients is hospitalized.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
  • Leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • Engages with the transitional care team and other resources for patients who cannot come to the office.
  • Takes full accountability for patient care and outcomes and seeks specialist consultation as needed.
  • Supervises, collaborates with, participates in or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions.

Leadership Development & Additional Leadership Duties

  • Demonstrate mastery of the Core Model and pursue partnership attainment within the first two years.
  • Assist the Associate Center Clinical Director and/or Market Clinical Director in onboarding new providers.
  • Shadow current Associate Center Clinical Director and/or Market Clinical Directors for additional leadership insights.
  • Engage in leadership development programs to prepare for an Associate Center Clinical Director role.
  • Lead weekly Super Huddle (SH) and Transforming Care Meeting (TCM) and support Center Operations Director when needed.
  • Assist Center and Market leadership in identifying and planning key clinical outcomes and coaching other providers.
  • Perform other duties as assigned and modified at the manager’s discretion.

Knowledge, Skills, and Abilities

  • Advanced-level business acuity.
  • Expert knowledge of general/core job-related functions and procedures.
  • Proficiency in Microsoft Office Suite and other company applications.
  • Willingness to travel locally, regionally, and nationally as required.
  • Openness to commute to neighboring centers or relocate to other markets.
  • Demonstrates emotional intelligence and effective communication skills.
  • Ability to build trust, provide specific feedback, and manage time and priorities.
  • Fluency in English (verbal and written); other languages as required by some centers.
  • Ability and willingness to work evenings, weekends, and holidays as needed.
  • Independent judgment and decision-making.

Education and Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
  • Current active MD licensure in the state of employment required.
  • Minimum of 1-year clinical experience in a geriatric, adult, or family practice setting preferred.
  • Prior leadership experience (e.g., Chief Resident, fellowship, or equivalent).
  • Board certification in the terminal specialty; if not certified, board eligibility is required.
  • Maintenance of board certification through MOC, CME, or exam retesting.
  • Current DEA number for schedule II-V controlled substances.
  • Basic Life Support (BLS) certification within the first 90 days of employment.

Pay Range

$221,141 - $315,915 Salary
