ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while fostering a positive culture of love, accountability, and passion.

  • PCPs lead a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator
  • PCPs engage with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)
  • PCPs have an active role in the management of their center and help cover for other providers

Responsibilities

  • Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
  • Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
  • Engage with the hospitalist whenever one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Lead their care team for patients able to come to the office
  • Engage with the transitional care team and others for patients unable to come to the office
  • Take an active role in recruiting patients for the center and additional providers for the company
  • Supervise, collaborate with, participate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP's actions

Compensation

  • Salary: $221,141 - $315,915
  • This position may also be eligible for bonuses or commissions

Requirements

  • Ability to function both independently and in collaboration with other health care professionals
  • Adherence to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures
  • Availability and accessibility for patients to build trust, including being open for walk-in visits and answering phone calls and messages in a timely manner
  • Service Orientation: provide care as they would want for a family member or themselves
  • Evidence Based Medicine: remain updated on evidence-based medicine, lifestyle, and nutrition interventions
  • Physician Leadership: continuously work to develop and improve leadership skills
