Primary Care Physician (PCP) Core Responsibilities

  • Accountability for outcomes: Provide strong clinical care and cost‑effective treatment for a panel of up to 450 patients; build trusting relationships and facilitate behavior change.
  • Health coaching: Set short‑ and long‑term health goals with patients, partner with them, and frequently follow up to improve health outcomes.
  • Simplify and prioritize: Make action steps clear for patients and for care teams to support performance goals.
  • Independent patient care: Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document care for patients with acute and chronic illnesses.
  • Care team leadership: Lead a care team (medical assistant, care facilitator, care coordinator) for patients available for office visits.
  • Transitional care coordination: For patients unable to visit the office, coordinate with hospitalists, transitional care teams, case managers, and other resources.
  • Center management and coverage: Assist in the management of the center, cover for other providers as needed, and support recruiting efforts.
  • Advanced Practice Practitioner (APP) oversight: Supervise, collaborate, and remain accountable for APP actions while employed with the company.
  • Other duties: Perform additional tasks as assigned and modified by management.

Completion of Metrics and Goals

Metrics such as patient admissions per thousand, appointment slots (18‑21 per day), CGCAHPS, clinical gaps closure, and medical cost measures are regularly measured and PCPs set goals for each metric in partnership with center and market leadership.

  • Availability and accessibility for patients; open for walk‑in visits and answer phone calls/messages promptly.
  • Service orientation: Provide care reflecting what one would give a family member.
  • Evidence‑based medicine: Stay current with clinical, nutritional, and lifestyle interventions.
  • Leadership: Continuously develop leadership skills for patients, teams, center, and company.
  • Quality focus: Work toward high‑quality outcomes and systematically improve practice and operations.
  • Influence: Compensate and influence patients, teams, and self to achieve best outcomes.
  • Self‑care: Prioritize physical, mental, social, and spiritual wellness.
  • Technology proficiency: Use Microsoft Office Suite and related software.
  • Travel readiness: Travel locally as needed, and nationally for initial training and occasional regional recruiting.
  • Language: Fluency in English (verbal and written); other languages may be required for specific centers.
  • Independent judgment: Make independent decisions related to patient care.

Education and Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
  • State medical license (or ability to obtain one) in the state(s) of employment.
  • Board certification in the relevant specialty preferred; board eligibility required.
  • Maintain board certification via MOC, CME or retesting.
  • DEA number for schedule II‑V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days.

Compensation

Pay Range: $221,141 – $315,915 annually. Final compensation may vary based on experience, education, geography, and other factors. Bonus or commission eligibility may apply.
