ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while also fostering a positive culture within the medical centers.

  • PCPs engage with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)
  • PCPs have an active role in the management of their center

Responsibilities

  • Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
  • Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed care
  • Engage with the hospitalist whenever one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Lead their care team for patients able to come to the office
  • Engage with the transitional care team and others for patients unable to come to the office
  • Help cover for other providers who may be out for various reasons
  • Take an active role as needed in recruiting patients for the center and additional providers for the company

Compensation

  • Salary: $221,141 - $315,915
  • This position may also be eligible for bonuses or commissions

Requirements

  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
  • Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written
