ChenMed is seeking a Primary Care Physician (PCP) to join their physician-led and mission-driven organization, focused on transforming healthcare for seniors. This role involves delivering affordable, VIP care to a panel of up to 450 patients, emphasizing strong clinical outcomes and cost-effectiveness. The PCP will act as a health coach, building trusting relationships and guiding patients toward improved health outcomes.

  • Accountability for outcomes for each patient in their panel of up to 450 patients.
  • PCP acts as a health coach, helping patients set short and long-term health goals.
  • PCP simplifies and prioritizes appropriately for behavior change and leading care teams.
  • Metrics regularly measured include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures.
  • PCP will have an active role in the management of their center and will help cover for other providers.
  • Each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Takes full accountability for patient care and outcomes, seeking consultation from specialists when needed while remaining responsible for detailed patient care.
  • Engages with the hospitalist whenever one of their patients is in the hospital.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • Leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • Engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources for patients unable to come to the office.
  • Adheres to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
  • Performs other duties as assigned and modified at manager’s discretion.

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
  • Board Eligibility is required
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
  • Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
  • Must have a current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
  • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written
