ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join their rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while also fostering a positive culture within the medical centers.

Practice Info

  • Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients
  • Acts as a health coach, helping patients set short and long-term health goals and frequently following up on those goals
  • Simplifies and prioritizes appropriately for actionable behavior change for patients and when leading care teams
  • Metrics regularly measured include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
  • Works closely with managers and medical directors to ensure compliance with guidelines and participates in risk and quality management programs, clinical meetings, and other meetings
  • Leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
  • Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
  • Engages with the hospitalist whenever one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Engages with the transitional care team and others (case managers, acute and transitional-care physicians, and other resources) for patients unable to come to the office
  • Has an active role in the management of their center and helps cover for other providers
  • Takes an active role as needed in recruiting patients for the center and additional providers for the company
  • Performs other duties as assigned and modified at manager’s discretion

Compensation

  • Salary: $221,141 - $315,915
  • This position may also be eligible for a bonuses or commissions

Requirements

  • Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written
