We are seeking a Board Eligible or Board Certified Family Medicine or Internal Medicine Physician to become part of a collaborative care team. This role focuses on improving health outcomes through preventive care, chronic disease management, and coordinated treatment.

Responsibilities
- Deliver comprehensive primary care services for adult patients
- Diagnose and manage acute, chronic, and complex medical conditions
- Interpret diagnostic tests and coordinate specialty referrals when appropriate
- Collaborate with interdisciplinary care teams to optimize patient outcomes
- Promote preventive care, wellness, and chronic disease management
- Participate in quality improvement initiatives and patient engagement programs
- Maintain accurate and timely electronic medical record documentation
Compensation
- $211,369 - $317,053 annually

Shift & Schedule

Requirements
- MD or DO from an accredited medical school
- Active Arizona medical license (or eligibility to obtain)
- Current DEA registration
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Passion for delivering value-based, patient-centered care
- Excellent communication and collaborative care skills
