ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician to join its rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The ideal candidate will be outcomes-focused, entrepreneurial-minded, and dedicated to building trusting relationships with patients to achieve better health outcomes.

Practice Info

  • Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in a panel of up to 450 patients
  • Acts as a health coach, helping patients set and achieve short and long-term health goals
  • Simplifies and prioritizes for actionable behavior change for patients and performance goals for care teams
  • Regularly measured metrics include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
  • Leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients
  • Engages with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound)

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
  • Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
  • Engages with the hospitalist whenever one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Has an active role in the management of their center and helps cover for other providers
  • Takes an active role in recruiting patients for the center and additional providers for the company
  • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP
  • Performs other duties as assigned and modified at manager’s discretion

Compensation

  • PAY RANGE: $221,141 - $315,915 Salary
  • This position may also be eligible for a bonuses or commissions

Shift & Schedule

  • Flexible to work evening, weekends and/or holidays as needed

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
  • Board Eligibility is required
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
  • Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
  • Must have a current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
  • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written
