Responsibilities
- Perform comprehensive patient evaluations, including history, physical examination, and diagnostic interpretation
- Diagnose and manage acute, chronic, and complex medical conditions
- Develop individualized treatment plans and monitor patient progress
- Order, interpret, and follow up on diagnostic tests (labs, imaging, PFTs, cognitive screenings, cardiac assessments, etc.)
- Provide preventive care, including immunizations, screenings, and lifestyle counseling
- Manage medication therapy, including prescribing, monitoring, and reconciliation
- Participate in care gap closure, chronic disease management, and preventive health initiatives
- Utilize evidence-based guidelines to improve patient outcomes and reduce avoidable hospitalizations
Compensation
- Compensation Range: $172,938 to $259,407
- The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting
- Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs
