ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join their rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while fostering a positive culture of love, accountability, and passion.

- Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients
- PCP acts as a health coach, helping patients set short and long-term health goals
- PCP simplifies and prioritizes appropriately for behavior change and leading care teams
- Metrics regularly measured include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
- PCP will lead their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for office patients
- For patients unable to come to the office, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources
Responsibilities
- Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
- Takes full accountability for patient care and outcomes, seeking specialist consultation when needed but remaining responsible for detailed care
- Engages with the hospitalist whenever one of their patients is in the hospital
- Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
- Leads their care team
- Has an active role in the management of their center and helps cover for other providers
- Takes an active role as needed in recruiting patients for the center and additional providers for the company
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP
Compensation
- PAY RANGE: $221,141 - $315,915 Salary
- Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors
- This position may also be eligible for a bonuses or commissions
