ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join their rapidly expanding team. This role focuses on providing VIP care to seniors, aiming to transform healthcare for the neediest population. The PCP will be accountable for patient outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients, while also fostering a positive culture within the medical centers.

  • PCP leads a care team consisting of care promoter (medical assistant), care facilitator, and care coordinator
  • Engages with transitional care team for patients unable to come to the office (in hospital, SNF, LTC, or homebound)

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient
  • Takes full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
  • Engages with the hospitalist when one of their patients is in the hospital
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
  • Leads their care team for patients able to come to the office
  • Has an active role in the management of their center and helps cover for other providers
  • Takes an active role as needed in recruiting patients for the center and additional providers for the company
  • Performs other duties as assigned and modified at manager’s discretion

Compensation

  • PAY RANGE: $221,141 - $315,915 Salary
  • This position may also be eligible for a bonuses or commissions

Requirements

  • Proficient in Microsoft Office Suite products (Word, Excel, PowerPoint, Outlook) and other relevant software
  • Ability and willingness to travel locally as needed in their market, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes
  • Fluency in English, verbal and written (fluency in other languages may be required for some centers)
  • Requires use and exercise of independent judgment
