Primary Care Physician

Job Overview

Conviva Primary Care, a subsidiary of Humana Inc., is looking for a Primary Care Physician at our clinic in Deerfield Beach, Florida. The role involves hands‑on patient care, addressing social determinants of health, and focuses on senior‑focused primary care.

Responsibilities

• Report to the Associate Medical Director.
• Work onsite and in‑person at the South Daytona clinic.
• Provide outpatient medicine, continuity of care, health maintenance, and disease prevention.
• Maintain medical histories and records.
• Refer patients to specialists.
• Influence clinic department strategy.
• Make clinical decisions on moderately complex to complex issues with latitude in best approaches.
• Participate in Humana/Senior Bridge’s tuberculosis screening program (TB screening required).

Schedule: Monday‑Friday, 8:00 am–5:00 pm, see 12‑15 patients per day with 45‑minute appointments; 100 % of time clinically focused.

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine.
• Board certified or board eligible in Geriatric Medicine, Family Medicine, or Internal Medicine.
• Valid, unrestricted license in the state of desired employment (or willingness to obtain).
• Current, unrestricted DEA license.

Preferred Qualifications

• Minimum two years of experience.
• One year experience managing Medicare Advantage panel.
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Fluency in Spanish and English.

Compensation & Benefits

• Base salary: $255,000 – $298,500 per year (full time, 40 hours per week).
• Eligible for a bonus incentive plan based on company/individual performance.
