Primary Care Physician

ChenMed is seeking a Primary Care Provider for their North Miami Beach clinic to deliver direct patient care as part of a clinical operations team. This role involves comprehensive patient management, from assessment and diagnosis to developing care plans and providing education, primarily in an ambulatory setting. The ideal candidate will contribute to transforming healthcare for seniors through a collaborative, physician-led approach.

• Functions independently as a primary care practitioner as part of a patient care team.

Responsibilities

• Provide direct patient care and assessments primarily in the ambulatory health care center setting, or occasionally in acute care, nursing homes, skilled nursing facilities (SNF) and home settings.
• Perform geriatric assessment, medical history, physical exam, diagnosis and treatment, and develop the plan of care.
• Provide health education, specialty referrals, case management referrals, and follow-up.
• Document clearly according to ChenMed standards for quality, service, productivity and teamwork.
• Participate in clinical rounds and conferences.
• Independently assess acute and non-acute clinical problems.
• Write admission, transfer and discharge orders; order and interpret appropriate laboratory and diagnostic studies; order appropriate medication and treatments; refer patients for consultation when indicated.
• Initiate arrangements and write orders for discharges and complete appropriate paperwork.

Compensation

• $238,832 - $341,189 Salary.
• This position may also be eligible for a bonuses or commissions.
• Our successful physicians will full patient panel are earning at or above the 90th percentile of MGMA.

Shift & Schedule

• Monday – Friday work schedule; Clinic Hours 7:30a – 5:00p.

Requirements

• Current licensure in the state of practice is required.
• Certification in Basic Cardiac Life Support required.
• Board certification in Internal Medicine or Family Medicine is preferred, required to successfully achieve Board certification within 2 years.
• Must have a current DEA number for schedule II-V controlled substances.
• Prefer minimum of one (1) year clinical experience in geriatric, adult of family practice setting.
• Unrestricted Florida MD or DO License.
• Board Certified in Internal Medicine or Family Practice.
• Fellowship in Geriatric is a PLUS.
• At least 1 year of Primary Care experience preferred.
• Managed Care Experience preferred.
