- Provide care for patients with acute and chronic illnesses encountered in the older adult patient
- Take ownership of patient care and outcomes, seeking consultation from specialists when necessary
- Help drive innovation in patient health by acting as a health coach and assisting patients in setting health goals
- Collaborate cross-functionally to simplify and prioritize care for patients, ensuring actionable behavior changes
- Lead efforts to manage their care team effectively, ensuring high-quality patient care and compliance with guidelines
Compensation
- Salary: $221,141 - $315,915
- This position may also be eligible for bonuses or commissions

Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required
- Current, active State Medical License for the state(s) in which the candidate will be working is mandatory
- Proficiency in Microsoft Office Suite products including Word, Excel, PowerPoint, and Outlook
- Experience with evidence-based medicine and a commitment to continuous improvement in patient care is essential
