The Primary Care Physician (PCP) In Our Organization Demonstrates
- Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviours.
- Coaching for health: The PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with the patient to work toward the goals, and frequently following up on those goals on the path to improved health.
- Simplifying for action: The PCP simplifies and prioritises appropriately so that behaviour change is more actionable for the patient and for their care team.
We are an outcomes‑focused, value‑based organization. For each PCP’s patient panel, metrics such as patient admissions per thousand, utilisation of 18–21 appointment slots per day, CGCAHPS, clinical gap closures, and medical cost measures are regularly measured to help PCPs become successful and reach partnership status. Each PCP will have goals for these metrics and is expected to work toward targets with centre and market leadership as well as care teams. Culture is very important; PCPs are expected to champion a positive culture of love, accountability, and passion alongside centre leadership.
The PCP must function both independently and collaboratively with other health‑care professionals. The PCP will work closely with managers and medical directors to ensure compliance with guidelines and participate in risk and quality management programs, clinical meetings, and other activities that promote patient health and company goals. The PCP will adhere to strict departmental goals, standards of performance, regulatory compliance, quality patient‑care compliance, and policies and procedures.
Essential Job Duties/Responsibilities
- Independent care of patients with acute and chronic illnesses encountered in older adults.
- Full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in the detailed care of the patient.
- Engagement with the hospitalist whenever a patient is in the hospital, regardless of hospitalist affiliation.
- Assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
- Leadership of a care team consisting of a medical assistant (care promoter), a care facilitator, and a care coordinator for office‑visiting patients.
- For patients unable to come to the office—whether in hospital, SNF, LTC, or homebound—the PCP will engage with the transitional‑care team, case managers, acute and transitional‑care physicians, and other available resources depending on the market.
- Active management of their centre and coverage for other providers as needed; recruiter of patients and additional providers for the company.
- Performs other duties as assigned and modified at manager’s discretion.

- Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally/nationally for recruiting or training.
- Fluency in English, verbal and written; additional language fluency may be required for certain centres.

Pay Range
$221,141 – $315,915 Salary
The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
