Primary Care Physician

ChenMed is seeking a Primary Care Physician (PCP) to join its rapidly expanding team, dedicated to transforming healthcare for seniors. This physician-led organization focuses on providing affordable, VIP care to its patients, aiming to be America's leading primary care provider. The role emphasizes accountability for patient outcomes, health coaching, and simplifying care for actionable behavior change within a value-based care model.

Practice Info

• Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in a panel of up to 450 patients
• PCP acts as a health coach, helping patients set short and long-term health goals
• PCP simplifies and prioritizes appropriately for behavior change and leading care teams
• Metrics regularly measured include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
• PCP leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office patients
• For patients unable to come to the office (in hospital, SNF, LTC, or homebound), PCP engages with the transitional care team and others including case managers, acute and transitional-care physicians

Responsibilities

• Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient
• Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for detailed patient care
• Engage with the hospitalist whenever one of their patients is in the hospital
• Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs
• Lead the care team
• Have an active role in the management of their center and help cover for other providers
• Take an active role as needed in recruiting patients for the center and additional providers for the company
• Supervise, collaborate with, participate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the actions of the APP

Compensation

• $214,700 - $306,714 Salary
• Outcomes-focused, value-based organization with metrics measured to help PCPs become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures
