• msDevelop strong transitional care plans and able to help keep track of paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home car
• e.Educate members on chronic disease progression and the need for advanced directives where applicab

Qualifications of the Lead Primary Care Physician /

• MD:Current Unrestricted lice
• nseMust have a minimum of 3 years full time experience practicing as a board-certified physic
• ianMust have 3 years lead experience , Must have recent experience working in Primary C
• areMust be comfortable prescribing opioi
