ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on delivering VIP care to seniors, emphasizing accountability for patient outcomes, health coaching, and simplifying care for actionable behavior change. The PCP will contribute to transforming healthcare for the neediest population by providing comprehensive, outcomes-focused care.

- Focuses on full-risk Medicare Advantage patients
- Manages a patient panel of up to 450 patients
- Utilizes 18-21 appointment slots per day (new patients count for 2 slots, follow-up patients 1 slot)
- Works collaboratively with center and market leadership as well as care teams
- Expected to champion a positive culture of love, accountability, and passion
Responsibilities
- Independently assess acute and non-acute clinical problems, perform and document physical assessments and patient histories, analyze trends in patient conditions, and develop, document, and implement patient management plans.
- Plan patient care based on in-depth knowledge of the specific patient population and/or protocol, anticipating and identifying physiological and/or psychological problems, considering cultural background, understanding, personality, and support systems.
- Write admission, transfers, and discharge orders.
- Order and interpret appropriate laboratory and diagnostic studies.
- Order appropriate medication and treatments.
- Refer patients for consultation when indicated (e.g., dermatology, neurology, ophthalmology, endocrine, surgery, intensive care, infectious disease, hematology, psychiatry, social service, dietary).
- Document through in-depth progress notes and summaries.
- Participate in patient care rounds and conferences, communicate patient management strategies to the patient care team, and collaborate with multidisciplinary team members.
Compensation
- $221,141 - $315,915 Salary
- This position may also be eligible for a bonuses or commissions.

Requirements

- Basic computer skills and some knowledge of Microsoft Office Suite
- ARNP or similar advanced degree in Nursing required
- Current Nurse Practitioner Certification in the State of practice required
- Board certification by AANP or ANCC is preferred but may be required for certain States
- Current DEA number from the DEA for schedule II-V controlled substances may be required based on State of practice
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
