Responsibilities
- Provide comprehensive primary care services for adult patients
- Deliver patient-centered care with a focus on prevention, chronic disease management, and wellness
- Accurately document patient encounters and support value-based care initiatives through quality coding and documentation
- Collaborate with an interdisciplinary team to provide coordinated, high-quality care
- Participate in population health initiatives and identify opportunities to improve patient outcomes
- Build long-term relationships with patients while delivering compassionate, evidence-based care
- Effectively identify and document risk-adjustment diagnoses
- Support patients through preventive care and chronic disease management
Compensation
- Compensation: $200,000-$225,000 annually, depending on experience

Shift & Schedule
- Progress toward an average of approximately two patient visits per hour while maintaining a high standard of care

Requirements
- Board Certification in Family Medicine or Internal Medicine
- Significant clinical experience in a primary care setting
- A passion for patient-centered care
- Experience with-or enthusiasm for-a value-based care model
- Strong electronic medical record (EMR) documentation skills
- Excellent communication and interpersonal abilities
- A collaborative, team-oriented mindset
