Primary Care Physician

Job Summary

Family Medicine Physician in Bardstown, KY. Provide comprehensive primary care for patients of all ages, including preventive care, acute illness treatment, diagnosis and treatment of illness, management of chronic conditions, and referrals to specialists. Serve as the primary healthcare provider across the lifespan, fostering long‑term relationships and guiding patients through their healthcare journey.

Responsibilities

• Provide full‑spectrum primary care, including preventive care, acute illness treatment, and management of chronic diseases (e.g., diabetes, hypertension).
• Deliver wellness examinations, health screenings, and immunizations.
• Counsel patients on diet, lifestyle choices, and disease management.
• Order, perform, and interpret diagnostic tests such as laboratory work and imaging.
• Offer guidance, counseling, and education on wellness, disease management, and medication adherence to patients and families.
• Maintain accurate, timely, and detailed medical records using EPIC.
• Provide and document direct treatment, including ordering medications, referrals, and following up on specialist consultations.
• Participate in primary care division activities such as peer review programs, quality metric improvement, and policy development.

Job Requirements

• Board‑eligible/Board‑certified.
