- Collaborate with APPs and an interdisciplinary team to manage acute and chronic conditions
- Improve population health and deliver high-quality, value-based care
- Mentor APPs and support clinical quality initiatives through leadership opportunities
Compensation
- $253,643 - $380,464 base salary
- Bonus opportunity
- Quarterly Incentive Plan (up to 15% of quarterly earnings)
- Long-Term Incentive Eligible through the Mosaic Health Profit Shares Program

Qualifications
- MD or DO
- Active unrestricted state medical license
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- DEA and BLS certification
- ACLS preferred
- Experience in primary care, chronic disease management, value-based care, and population health
