Primary Care Physician

ChenMed is a physician‑led, mission‑driven primary care organization focused on Medicare Advantage seniors. We seek a Primary Care Physician (PCP) who will deliver high‑quality, value‑based care to a panel of up to 450 patients and support our outcomes‑focused culture.

Responsibilities

  • Provide independent care for acute and chronic illnesses in older adults, including assessment, diagnosis, treatment, education, health promotion and documentation.
  • Take full accountability for patient outcomes and seek specialist consultation as needed while remaining involved in detailed care.
  • Engage hospitalists for in‑hospital patients and coordinate care with transitional teams for homebound or facility patients.
  • Lead a care team comprising care promoter (medical assistant), care facilitator, and care coordinator.
  • Participate in risk and quality management programs, clinical meetings, and regulatory compliance activities.
  • Support center management and cover for other providers as needed.
  • Assume an active role in recruiting patients and additional providers.
  • Supervise, collaborate with, or function within a practice agreement with an Advanced Practice Practitioner (APP) and be accountable for the APP’s actions.
  • Perform other duties as assigned by management.

Qualifications and Abilities

  • Availability and accessibility to patients, including walk‑in visits and timely responses to calls and messages.
  • Strong service orientation: provide care that a family member would desire.
  • Knowledge of evidence‑based medicine, including lifestyle and nutrition interventions.
  • Willingness to develop and demonstrate physician leadership skills.
  • Commitment to quality improvement and achievement of performance metrics.
  • Influence patients, teams, and self to achieve optimal outcomes.
  • Engage in self‑care to sustain personal and professional well‑being.
  • Proficient in Microsoft Office Suite and other clinical information systems.
  • Ability and willingness to travel locally for clinical duties and occasionally nationally for training or recruiting.
  • Fluency in English, verbal and written. Fluency in other languages as required will be specified.

Pay Range

$221,141 – $315,915 per year (salary).
