Primary Care Physician (PCP)

The Primary Care Physician at ChenMed demonstrates accountability for patient outcomes, serves as a health coach, and simplifies care to promote behavior change. PCPs manage panels of up to 450 patients and work within a value‑based, outcomes‑focused organization.

Key Responsibilities

  • Provide care for acute and chronic illnesses in older adult patients.
  • Take full accountability for patient care and outcomes, seeking specialist consultation as needed while remaining involved in detailed care.
  • Engage with the hospitalist for patients admitted to any hospital, regardless of affiliation.
  • Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document patients with acute and complex chronic needs.
  • Lead the care team consisting of a medical assistant, care facilitator, and care coordinator for office‑based patients.
  • Coordinate with transitional care teams for patients unable to come to the office (hospital, SNF, LTC, or homebound).
  • Participate in center management, cover for other providers, and assist in recruiting patients and additional providers.
  • Supervise and collaborate with an Advanced Practice Practitioner, maintaining accountability for the APP’s actions.
  • Perform additional duties as assigned or modified by management.

  • Commitment to high quality outcomes and continuous improvement of practice and center operations.
  • Influence patients and teams to achieve optimal outcomes.
  • Self‑care to maintain personal wellness.
  • Proficiency in Microsoft Office Suite and related software.
  • Willingness to travel locally on the job, nationally for initial training, and regionally or nationally for recruiting or training.
  • Fluency in English, verbally and written; additional language proficiency as required.
  • Independent judgment and flexibility to work evenings, weekends, and/or holidays as needed.

Education & Experience Criteria

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Ability to obtain or currently hold an active State Medical License for the state(s) of practice.
  • Preferred board certification in the specialty; board eligibility required.
  • Maintenance of board certification through MOC, CME, or retesting as required.
  • Current DEA number for schedule II‑V controlled substances.
  • Basic Life Support certification from the American Heart Association or American Red Cross within the first 90 days.

Pay Range

$221,141 – $315,915 annual salary, subject to experience, education, geographic location, and other factors. This position may also be eligible for bonuses or commissions.
