Primary Care - Nurse Practitioner / Physician Assistant

Primary Care Physician – McKinleyville Community Health Center

We are searching for a Primary Care Physician to join our integrated care team at McKinleyville CHC in McKinleyville, California. The role involves assessing and evaluating the health care needs of patients, performing diagnostic and therapeutic procedures, and managing patient care within our multidisciplinary, team-based model.

Responsibilities

• Assess and evaluate patient health care needs.
• Perform diagnostic and therapeutic procedures.
• Institute and manage care strategies for patients.
• Collaborate with behavioral health, dental, and pharmacy services.
• Provide patient education and continuity of care for a diverse underserved population.

Qualifications

• Diploma from an accredited program with credentialing through the American Nurses Association or Physician Assistant Association.
• Current, unrestricted California license.
• Current DEA registration.
• Current Board Certification in specialty of practice (or board‑eligible if a newer graduate).
• Current CPR/BLS certification.
• Valid California driver’s license with an insurable driving record.

Compensation and Benefits

• Annual salary range: $148,627.20 to $168,553.60.
