Your Responsibilities Include:

  • Delivering personalized high‑quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
  • Delivering dementia‑informed, relationship‑based care across the continuum of care.
  • Conducting comprehensive assessments and developing tailored care plans for older adults.
  • Managing chronic conditions and performing care with skill and empathy for patients and their families.
  • Collaborating with cross‑functional partners – including palliative, psych, and neuro – to deliver coordinated, whole‑person care.
  • Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintaining consistency in documentation, follow‑through, and patient communication.

What You’ll Bring

  • Active, unencumbered NP or PA license in the state you will be practicing.
  • Minimum of 1‑2 years of clinical experience (geriatrics or primary care preferred).

  • Valid driver’s license and reliable transportation.

Pay Range for Posted Region: $140,000 USD - $200,000 USD
