- $119,313 – $178,968annually, plus annual incentive opportunities and sign-on bonus.

Requirements
- NP Board Certification withprimary care, family medicine, or internal medicine background
- Current, valid andunrestricted ARNP license and valid DEA number
- English/Spanish bilingualproficiency is an essential job function
- Minimum 3-5 years of primarycare experience
- Minimum 2+ years with casemanagement emphasizing health prevention and population health management
- Documentation of proof of anegative TB test in the last 12 months before start-date.
- Team-based care experience
- Experience working in EMRsystems
- Experience with GoogleWorkspace
