Inspire Health Alliance seeks a Primary Care Nurse Practitioner for the Eureka VA Outpatient Clinic to deliver patient-centered primary care to Veterans in a rural setting, within the PACT model and telehealth when appropriate.

The PCNP provides comprehensive exams, preventive care, chronic disease management, care coordination, and collaborates with an interdisciplinary team to ensure VA standards and continuity of care.
