Primary Care NP: Team-Based Health & Wellness Leader

Vera Whole Health, Inc. is seeking a provider to deliver evidence-based primary care to a diverse patient population in a team-based setting.

You will assess needs, develop and implement biopsychosocial care plans, and collaborate with the care team to improve health outcomes across the population. The role includes 12-16 patient panels daily, involvement in quality improvement initiatives, and participation in after-hours on-call rotation.
