Job Details:

• Join an established family medicine practice caring for patients from birth through adulthood in a 100% outpatient setting


• Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA)


• Full-time position with a four-day work week option available


• Weekdays only schedule with no call required


• Serve a diverse, multicultural patient population including many Spanish-speaking families


• Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans


• Develop plans for further evaluation of health needs, including referrals to specialists and other health team members


• Provide appropriate treatment for problems within scope of practice and refer outside cases to physician as needed


• Work collaboratively with practice physicians and care team members


• Unrestricted State of Connecticut Medical License required (or eligible)


• Minimum of 1+ years of experience in a primary care or similar setting required
